Program Evaluation

If you have participated in the program and would like to provide a formal evaluation, please use the online evaluation form.

Alternatively, you may fill out and mail in the form included in the Viewers Guide and/or the CME packet. Evaluation forms should be mailed to:

If you are sending an evaluation only, there is no need to include any payment for processing. Payment is required only when sending the evaluation in along with the Post-test for CME credit.

Copies of this program (1 VHS videotape with printed Viewers Guide) are available from The Hastings Center for $15.00 each, plus shipping and handling of $3.50 for the first copy and $0.50 for each additional copy shipped at the same time to the same address, for locations within the U.S. There is a shipping and handling charge of $8.50 for the first copy shipped to destinations outside of the United States and $2.00 for each additional copy shipped to the same foreign destination at the same time. Orders must be accompanied by a check or money order, (payable in U.S. currency, drawn on a U.S. bank), or charged to MasterCard, VISA or American Express. Please make checks payable to The Hastings Center.

CME

The deadline for submitting materials for CME credit has expired as of June 30, 2004. Materials received after that date will not be processed. All CME is handled by the College of Physicians & Surgeons of Columbia University in New York City. For more information on submitting materials for CME credit, please click here.
